| Suggestions for a Successful Referral and Placement. 1. Call (919) 942-1625 and inquire about program openings while informing us of some general details on the child. Be sure and mention which program you're interested in your client being placed in Level II Therapeutic Foster Care, or Family Foster Care. 2. Application - print and complete the application including as much required information as you can obtain and fax, e-mail, or mail the initial packet with signatures. 3. Be available to provide additional forms or information so that our team can make an informed decision to place the client. 4. Once the application has been made, an appointment will be set up to meet face to face and determination if the home identified is a suitable match for both parties interested in placement. If the decision is made to move forward, consent forms will be completed at this intake meeting and a date for admission into the home will be set. |

| To All Interested in Placing a Child with Youth Quest, Youth Quest will celebrate our 13th anniversary July 1, 2011! We've dedicated the past decade to providing safe, high quality, evidence-based therapeutic foster care and residential treatment for youth. Youth Quest has a unique Level II therapeutic foster care program with an evidence-based treatment curriculum. These unique, structured yet nurturing homes foster life-long social, academic, and family/independent living skills for the scores of youth and families who have the opportunity to be helped at Youth Quest. While living in one of our licensed homes can be challenging at times as change is always tough, youth and families often remain in contact with our professional therapeutic foster parents and staff members for years after treatment has been completed to express gratitude for the many life skills these youth and families leave our program with helping them to live happier, healthier, and more productive lives. Our TFS Program or Therapeutic Family Services Program is a program with a proud beginning. With the desire to effectively serve just one youth by offering our unique model of care in a therapeutic foster home, and finding none in the area, we've opened up this program in 2003. Many youth have formed strong mentor and therapeutic relationships with their therapeutic family teacher providers offering them increased resilience. We look forward to the pending state changes proposed to transition therapeutic foster care or (TFC) with the new service called therapeutic family services (TFS). Our goal is to offer nothing short of the best therapeutic foster care services in North Carolina! Please let us know when we're accomplishing our goal, as well as if we ever fall short of this expectation? Your feedback is always appreciated. Sincerely, Emily K.
